An Old Vaccine, A New Hope for Alzheimer’s

The most surprising science story here is about the BCG vaccine. We have used it for 100 years to fight TB. Now, researchers at Massachusetts General Hospital think it might train the immune system to fight Alzheimer’s.

Here’s the mechanism. Alzheimer’s is linked to amyloid-beta plaques in the brain. The body’s immune cells, microglia, are supposed to clear them. But with age, they get sluggish. BCG seems to cause "trained immunity" — it puts innate immune cells on high alert. In the study of 23 older adults, two doses of BCG led to stronger immune responses in cerebrospinal fluid. In theory, that means better plaque clearance.

Analytically, this matters for 3 reasons.

First, cost. New anti-amyloid drugs like Leqembi cost lakhs per year and need infusions. BCG costs a few hundred rupees and has a century of safety data. If it works, even partially, it changes access. Second, timing. This is not presented as a cure. Dr. Basak calls it a "preventive measure". That fits the larger shift in Alzheimer’s research: intervene 10-15 years before symptoms. A cheap vaccine given in your 60s is a plausible public health tool. Third, risk and rigor. The study had no placebo group and only 23 people. That is tiny.

Immunology is also messy. "Training" the immune system can help, but it can also cause inflammation. We don’t yet know if BCG in the brain is net good or net harmful long-term. BCG is not new. The insight is. That is often how science moves — not with a moonshot, but by looking at old tools with new questions. The next step is clear: large randomized trials. Without them, this remains a promising clue, not a treatment. But the clue is important. If a TB vaccine can nudge the brain’s immune system, it opens the door to other repurposed drugs for neurodegeneration.

Analytical conclusion: Don’t expect BCG shots in neurology clinics next year. Do expect more funding for "trained immunity" and prevention trials. In a disease where we have failed for decades, even a small, cheap signal is worth chasing.
